Carlos Edwards scored an absolute screamer as Ipswich ended their Portman Road season in sensational style.
Luke Chambers heded in an Aaron Cresswell free kick, while Daryl Murphy showed great skill to turn and finish in the second half.
Frank Nouble twice had the ball in the back of the net but was ruled offside each time as the Blues produced some great attacking play.
Birmingham City pulled one back through substitute Chris Burke, which will disappoint the otherwise excellent defence.
Town hae gone from bottom of the table from November when McCathy took over, to a potential top 10 finish depending how they get on at Burnley next week.
